Coaching Client Intake Questionnaire Template 2026

Most coaching sessions that go sideways share one thing in common: the coach didn't know enough about the client before they started. A strong intake questionnaire — sent before the first session — gives you the intelligence to show up prepared, ask the right questions, and actually move the client forward from day one. Coaches who skip it often spend the first 30 minutes of a paid session collecting background that should have been gathered in advance. This post gives you a complete, copy-paste coaching client intake questionnaire template covering goals, challenges, communication style, and session expectations.

Why Your Intake Form Matters More Than the Discovery Call

Discovery calls are great for building rapport. But intake questionnaires do something a conversation can't: they give clients space to think. When someone fills out a written form, they slow down, commit their real answers to text, and arrive to the first session already doing the work. Coaches who use pre-session intake forms consistently report that clients arrive more self-aware, more focused, and more ready to move — because the form itself starts the reflection process before you've said a word.

An intake form also protects you professionally. If a client says three months in that "this isn't what I signed up for," their written goals, challenges, and expectations are on file in their own words. That's a document, not a memory.

The Complete Coaching Client Intake Questionnaire Template

This template is built into four sections of 4 questions each — short enough that clients complete it in under 10 minutes, specific enough that you walk away with real intelligence. Paste it into any form tool, or use ROXO Hub's built-in Forms & Waivers to attach it directly to your booking confirmation so it's sent automatically every time a new client books.

Section 1: Goals & Outcomes

  1. What is the #1 result you want to achieve through coaching? Be as specific as possible — "get more confident" is less useful than "land my first $10k client by August."
  2. Where do you want to be in 90 days? In 12 months? Describe what your life or business looks like when things are going the way you want.
  3. How will you know you've succeeded? What will be measurably or noticeably different?
  4. Have you worked with a coach before? If yes, what worked well — and what didn't?

Section 2: Challenges & Obstacles

  1. What is the biggest obstacle standing between you and your goal right now? If you already knew how to remove it, you would have.
  2. What have you already tried that hasn't worked? Be honest — this helps me avoid repeating what you've already ruled out.
  3. What tends to get in your way when you try to make a change? (Examples: perfectionism, time, self-doubt, money, fear of judgment from others.)
  4. On a scale of 1–10, how ready are you to take real action this week? There's no wrong answer — a 6 tells me something important, and so does a 10.
Warning: If a client rates their readiness 4 or below, address it before session one. Coaching someone who isn't ready to act is frustrating for both of you — and won't produce results either of you can be proud of.

Section 3: Communication Style

  1. How do you prefer to communicate between sessions? (Text / Email / Voice message / No contact between sessions — all are valid.)
  2. What time zone are you in, and which days and times work best for sessions?
  3. Do you prefer direct, straight-talking feedback — or a more exploratory, question-based coaching style?
  4. Is there anything about how you learn or process information that I should know? (Examples: need things in writing, prefer visual examples, process better when talking it through out loud.)

Section 4: Session Expectations

  1. How often do you want to meet? Weekly / Bi-weekly / Monthly.
  2. What would make a session feel like a complete waste of your time? Be honest — this shapes how I structure our work together.
  3. What do you need from your coach to feel fully supported?
  4. Is there anything personal or professional I should be aware of as we begin working together? You don't have to share anything you're not comfortable with, but the more context I have, the better I can serve you.
Pro tip: Add one final open field at the bottom: "Anything else you want me to know before our first session?" Some clients write three sentences. Some write three paragraphs. Both give you something to work with.

How to Send This Questionnaire Professionally

A Word document attached to an email technically works — but clients have to download it, fill it in, save it, and email it back. Most won't, especially on mobile. A link to a digital form has dramatically higher completion rates and takes clients 30 seconds to open. The format of your intake process signals something about how you run your business before the first session even starts.

The best practice for coaches charging $150–$500 per session: automate delivery entirely. Send the intake form the moment a new client books — no chasing, no follow-up, no remembering. If you're still emailing questionnaires manually to every new client, that's a workflow problem a better tool can solve in about 15 minutes of setup.

The Right Tool Makes This Easier

ROXO Hub was built for service-based solo businesses — including coaches who are tired of stitching together Google Forms, Calendly, and Venmo just to onboard one client. With ROXO Hub's Forms & Waivers feature, you build your intake questionnaire directly inside the platform and attach it to your booking flow. When a client self-books a session online, the form is sent automatically — no manual step required on your end.

Every client's intake responses are stored on their profile in ROXO Hub, alongside session history, notes, and payment records. You can review their answers in seconds before any call. You can optionally enable card-on-file storage or deposit collection at booking if you want no-show protection built in. Auto Reminders go out before each session so clients show up prepared. And your booking page is hosted on ROXO Hub's built-in Website Builder — no separate website tool needed.

All of that is included in one flat rate: $39.99/month. No form-builder add-on. No booking-link surcharge. No per-client fees.

Result: Your intake questionnaire becomes part of your automatic booking flow — clients complete it before the first session, and their answers are waiting on their profile when you open it.

FAQ

What should be in a coaching client intake questionnaire?

A strong coaching intake questionnaire covers four areas: the client's primary goal and what success looks like, the obstacles and patterns that have blocked them so far, their communication preferences and schedule, and their expectations for how sessions should run. Keeping it to 12–16 focused questions means clients complete it without dropping off halfway.

How long should a coaching intake form be?

Aim for 12–16 questions across 4 short sections — most clients complete this in 8–12 minutes. Forms that are too long get abandoned; forms that are too short don't give you enough to work with before the first session. Four sections of four questions each is the sweet spot for completion rate and usefulness.

When should I send the intake questionnaire to a new coaching client?

Send it immediately after booking — ideally automated so you don't have to remember. Ask clients to complete it at least 24 hours before the first session so you have time to review their answers and prepare. Platforms like ROXO Hub can trigger the form automatically the moment a client books online, removing the manual step entirely.

Can I use a digital form instead of a paper or Word document questionnaire?

Yes — and you should. Digital forms sent via a link have far higher completion rates than documents clients need to download, fill in, and email back. Most clients will open a link on their phone in under a minute. ROXO Hub's Forms & Waivers feature lets you build intake forms directly in the platform and attach them to your booking confirmation automatically.

Do I need a separate liability waiver with my coaching intake form?

Most coaches benefit from a separate consent and scope-of-practice document that clarifies what coaching is and isn't — not therapy, not medical advice. This is a distinct document from an intake questionnaire. ROXO Hub's Forms & Waivers feature lets you build and send both the intake questionnaire and any consent documents as part of the same digital onboarding flow.

What's the difference between a coaching intake form and a discovery call?

A discovery call is a verbal conversation used to assess fit before a client commits to working with you. An intake form is sent after the client has already signed up, to gather the detailed background information you need to coach them effectively. Both serve different purposes — the intake form is what gives you the intelligence to run a strong first paid session.

Stop Chasing Clients for Paperwork

With ROXO Hub, your intake questionnaire is sent automatically the moment a client books online — no manual follow-up, no email attachments.
