Section 3: Contraindications & Current Medications

Collect client preferences before the appointment — arrive with a plan, not a blank canvas.

Section 7: Consent, Aftercare Agreement & Signature
Every client must read, understand, and sign this section — including returning clients coming in for a touch-up.
- Client confirms all health history disclosed above is accurate and complete
- Client understands PMU results vary by skin type and individual healing response
- Client acknowledges a touch-up (typically 6–8 weeks post-procedure) may be required for optimal results
- Client agrees to follow all aftercare instructions provided at the appointment
